Dear CBS Lawrence Family,

We are pleased to announce that we are once again publishing a CBS Yizkor Memorial Book. The book will contain the Yizkor Tefilot and will be used at our Minyanim on Yom Kippur, Shemini Atzeret, Pesach and Shavuot. The book will also offer our members the opportunity to dedicate and memorialize loved ones by listing their names in the memorial section.

In order to participate, please complete this form, listing the names of those you wish to memorialize with a contribution of $36 per name. If you participated last year and want to repeat the same listings, there's an option for that. Click Here to see last year's Yizkor Book.

We are looking for a lead Yizkor Book sponsor at $3600. Please contact our Executive Director, Rand Levin, if interested. 516-569-3600 Ext. 102 or execdir@bethsholomlawrence.org.

There are several other sponsorship levels available as well (not including $36 memorial listings). Sponsors will be listed by level in the front of the book. Please note, one is not required to list names in order to be a sponsor (select None in the form below).

Deadline: Forms must be submitted by Monday September 15, 2025 allowing us enough time to print the books for Yom Kippur.
